[spoiler]As you can see, his eye began to bleed after the first attack, another one would just blow up his eye. So what would be the point of the attack if it failed? Itachi was a smart guy, I'm pretty sure he wouldn't just use it so that he could try it only once. He gave it to Sasuke to protect him, and help save the Leaf Village by killing Madara/Tobi[/spoiler]
